Checkbox not displaying on form?

I have created a sub form and the last field is a checkbox where the subscriber agrees to a a condition, howver the label is displaying but there's no checkbox. What am i missing?

Please provide a screenshot of the subscription module configuration?

Did you select article which should be linked to the checkbox? Only if you select the article, check box will be visible

Thanks for that, I assumed it was like a yes or no, there is no article so I have changed the field to a Yes/No drop down.
