jvstratum Hi, We recommend using a survey extension. We have an add-on available for Community Survey, which is documented here: https://docs.acymailing.com/addons/joomla-add-ons/community-surveys. Alternatively, you could use a form extension such as RSForm and use Acymailing to send the link to your subscribers.
