Unsubscribe admin email confirmation

Hello,
i cant find a way to have a notification email when a user unsubscribe for a list.
Thanks to help me on this.

OK, it work but... How can i put the username and email of the unsubscribed user ?

Because if i use the tag in the email template, i receive my username and email...
