Hi,
You can add the custom fields to both the subject and body of the confirmation email. Theres no need to use a custom field for the first name. You can use the core field and display the first name only by using this tag: {subscriber:name|part:first|ucfirst}
You're replying to a very old thread, which never makes sense. Lot's of things have changes since then and previous contributors are now receiving notifications about new replies for something that already has been solved.
